This form is required for all children participating in our drop-off events. It collects essential information, including your child’s details, emergency contact information, and a signed liability agreement. Completing this form ensures we can provide a safe and enjoyable experience for all participants. Please fill out and submit the form prior to the event date to secure your child(ren)’s participation.

Liability Waiver

I hereby for myself, my family and heirs, children/wards, executors, administrators and assigns do hereby release and forever discharge Soper Creek Wildlife Rescue & Ranch Inc., its directors and officers, owners, agents, principals, successors and assigns and all other persons or entities acting in any capacity on its behalf (collectively, SOPER CREEK WILDLIFE RESCUE) of and from all causes of action, claims or demands against Soper Creek Wildlife, that I and/or my child/ward/family ever had, now have, or can, shall or may hereafter have on account of any loss, including death, damage or injury sustained by me and/or my child/ward, or from any medical or therapy treatment advised or performed, or in respect of the loss of any equipment or opportunity used by me, arising from participation in this Soper Creek Wildlife event, whether caused by negligence or other actions/omissions of Soper Creek Wildlife or otherwise.
